The WHAT and WHY?
A long-term condition (LTC) review is an annual check-up that includes taking your vitals and having a blood test followed by a medication review to ensure the right treatment plan is in place for a healthier tomorrow!
An LTC review is to monitor and manage your condition(s) to ensure treatment is safe and effective. Also, to help prevent complications and hospital visits.
Won’t remember to book one every year? Use your birthday month as a reminder!

What gets checked?
Depending on your condition(s), this may include: a combination of blood tests, urine tests, blood pressure monitoring, physical checks such as height, weight and foot check.
At medication review your GP or Clinical Pharmacist will review the dosage, potential side effects, and discuss any changes to ensure the medications are working effectively.
